Novel Perioperative Pharmacologic Therapy Of Heart Failure
نویسنده
چکیده
Heart failure (HF) is a pathophysiological condition with an inability of the heart to fill or eject blood sufficient to meet metabolic demands. The etiology of HF appears evenly distributed between systolic and diastolic dysfunction and currently is the only cardiac condition with an increasing mortality rate. As the elderly population grows, so will the frequency of HF patients presenting to the hospital and operating room. An insult to the heart (ischemia, hypertension, virus, pregnancy, hemochromatosis, idiopathic) causes left ventricular (LV) dysfunction and a reduction in cardiac output with secondary neurohormonal activation (sympathetic nervous system; renin-angiotensin-aldosterone system or RAAS; vasopressin; endothelin; cytokines) and alterations in renal tubular sodium and water retention. This viscious cycle markedly alters left ventricular (LV) loading conditions and becomes a progressive disorder with LV remodeling, fibrosis, dilation, and failure.

Anesthetic management of the patient with low ejection fraction.
The number of patients with heart failure presenting for surgery continues to rise, and anesthesiologists are increasingly being called upon to provide quality, safe care in the operating room for patients with low ejection fraction (EF). Perioperative goals in the management of these patients include maintaining forward flow, promoting inotropy without inducing or exacerbating ischemia, and re...
متن کاملPharmacokinetics of the Perioperative Use of Cancer Chemotherapy in Peritoneal Surface Malignancy Patients
Background. The peritoneal surface is an acknowledged locoregional failure site of abdominal malignancies. Previous treatment attempts with medical therapy alone did not result in long-term survival. During the last two decades, new treatment protocols combining cytoreductive surgery with perioperative intraperitoneal and intravenous cancer chemotherapy have demonstrated very encouraging clinic...
متن کاملCardiac resynchronization therapy for congestive heart failure.
Cardiac resynchronization represents a novel therapeutic strategy for the treatment of congestive heart failure due to systolic dysfunction. Since its modest beginnings in the 1990s, cardiac resynchronization therapy has gained widespread acceptance as a useful adjunct to pharmacologic therapy for congestive heart failure. Randomized trials have consistently shown functional improvement in pati...
متن کاملMolecular signature of recovery following combination left ventricular assist device (LVAD) support and pharmacologic therapy.
AIMS A novel combination therapy consisting of a left ventricular assist device (LVAD) combined with pharmacologic therapy including the selective beta(2)-agonist, clenbuterol, has shown promise in restoring ventricular function in patients with heart failure. The aim of this study was to identify common genes and signalling pathways whose expression was associated with reversal of heart failur...
متن کاملPerioperative management of chronic heart failure.
Heart failure (HF) is one of the few cardiac conditions that is increasing. Despite a better understanding of how hormones and other signaling systems underlie the pathophysiology, and despite improved outcomes from pharmacologic therapy, many HF patients receive no effective treatment. Patients with HF commonly require medical diagnosis and management in operating rooms and critical care units...
